Usage;-
When to use
Serious respiratory tract anaerobic infections, serious skin and soft tissue infections, septicemia, intra-abdominal infections, infections of the female pelvis and genital tract
How to use
Severe anaerobic infections
Oral:
Adults: 150-300 mg every 6 hour. May increase to 450 mg every 6 hour in more severe infections. Max: 1.8 gm/day.
Children: 3-6 mg/kg every 6 hour. Children weight <10 kg should receive at least 37.5 mg every 8 hour.
Parenteral (IV):
Adults: 0.6-2.7 gm daily in divided doses, increased to 4.8 gm daily in very severe infections.
Children: >1 month: 15-40 mg/kg daily in divided doses. A minimum dose of 300 mg daily should be given regardless of body weight.
Prophylaxis of endocarditis
Parenteral (IV):
Adults: 600 mg 1 hour before dental procedure. 300 mg may be given by intravenous (IV) injection (over at least 10 minutes) for high-risk patients undergoing dental procedures involving general anaesthesia.
Toxic shock syndrome/Pelvic inflammatory disease
Parenteral (IV):
Adults: 900 mg every 8 hour, in combination with penicillin G or ceftriaxone.
Acne:
Adults: As 1% preparation: Apply a thin layer onto affected area twice daily.
Bacterial vaginosis
Adults: As pessary or 2% cream: 100 mg once nightly for 3-7 days.
Renal /Hepatic impairment: dosage adjustment may be needed in severe case.
Risk Warnings
Urticaria, rashes, contact dermatis, exfoliative, vesiculous dermatis, local irritation abdominal pain, oesophagtis,nausea, vomiting, diarrhoea, jaundice, liver abnormalities, eosinophilia, erythema,multiforme, thrombophloebitis, gasping syndrome (premature infants, neonates) due to preservative benzoyl alcohol in parenteral formulation, pseudomembranous colitis, azotemia, oliguria, proteinuria
Precautions
Hepatic, renal impairment, pregnancy, lactation, gastrointestinal (GI) disease, elderly, atopic patients, regular monitoring of blood counts, in conjuction with an biotic therapy

Hypersensitivity, meningitis as it has less penetration into CNS, pseudomembranous colitis
